Our office participates in most major health plans at Saland Vision. We are not, however, in network with any vision insurance plans. Check the list below to see if your health plan is included in the list of plans we accept. If you do not see your plan listed here, please call your insurance company to confirm whether or not your physician is currently in network with your plan, or call our office to find out if we are part of your insurance network. Please keep in mind that this list changes and is not comprehensive; it is included solely as a guide.
Please bring your insurance card and a photo ID to each visit.
Please remember insurance coverage is a contract between the patient and the insurance company. The ultimate responsibility for understanding your insurance benefits and for payment to your doctor rests with you.
If we do not participate with your insurance carrier, you may want to ask your carrier if you have out-of-network benefits. We will provide a detailed receipt that can be submitted for out-of-network medical or vision benefits. Any deductible or portion due from the patient will be due at the time of service.
A refraction is the process of determining if there is a need for corrective eyeglasses. It is an essential part of an eye examination and necessary in order to write a prescription for glasses.
Most medical insurance plans, including Medicare, do NOT cover routine refractions or routine eye examinations.
Our office fee for refraction is $75.00 and this fee is collected at the time of service in addition to any co-payment your plan may require.
It is your responsibility to make certain that the appropriate referrals are in place PRIOR to your visit. We can usually tell you whether your insurance company requires a referral. Please take the time to confirm with us that all necessary referrals have been received ahead of time. If your referral is not in place at the time of your visit, this will delay your visit. Further, if it is not possible to obtain the referral while you are waiting at our office, you will have the choice of: a) paying out of pocket for the visit, or b) rescheduling your appointment.
